SERVICES:
Pain Management & Rehab Associates offers complete medical,
diagnostic and treatment services for problems of pain, weakness, numbness, and physical
disability. Specialized care is available for arthritis, back pain, neck pain, muscle
problems, pinched nerves, sports injuries, work-related disability, and automobile
accident related trauma. In addition to the physical medicine and rehabilitation
physician, treatment may include other medically directed health care professionals. Our
treatment team may consist of psychologists, diagnostic technicians, physical therapists,
occupational therapists, speech pathologists and vocational counselors.

OFFICE HOURS & APPOINTMENTS:
Hours for patient appointments are Monday-Friday from 8:00AM-4:30PM.
Please call during normal business hours for an appointment. If calling after hours or if
all lines are busy, leave a voice mail message including a home and work phone or pager
number.
URGENT CARE & EMERGENCIES:
Same day appointments during usual
office hours are available for established patients with urgent outpatient medical needs.
When the office is not open, patients should contact their primary care physician or
utilize local emergency room services at our affiliated hospitals.
CALLING OUR OFFICE:
Our staff has been trained to handle problems or
questions received by phone. This enables your doctor to attend to patients with less
delays and interruptions. If your call requires consulting the physician, please call back
at the specified interval for your physician's response or prescription.
BILLING & INSURANCE: We request payment at time of service for the initial consultation,
office visits and services not covered by the patient's insurance plan. For participants
of Blue Cross/Blue Shield of Michigan, Medicare, and selected insurance benefit plans,
many hospital and office services are covered.
Patients are required to pay for their office visits,
insurance co-payments, and deductible amounts at time of service. Payments can be made by
cash, personal check, Visa, Mastercard or Discover . Services covered by insurance can be
billed to the insurance company if we have complete billing information.
